Founded in 1984, Heritage Sierra Medical Group (HSMG) joined Heritage Provider Network in 2008. We have created an atmosphere of wellness by promoting the idea that health is a function of mind and body.
We have diligently invested our time and resources in the development of programs and services responsive to the healthcare needs of today's patient while adapting to the increasingly diverse needs of tomorrow's healthcare agenda.

At Heritage Sierra Medical Group, our mission is to ensure fully coordinated, technology-driven patient care that holds our team to the highest level of service.
On-site services provided by our facility include urgent care, internal medicine, family medicine, pediatrics, lab draw, women's health, x-ray, specialist referral, urgent/walk-in care, and doctor-on-call after-hours.
Our Urgent Care Centers are capable of handling the vast majority of medical conditions. After initial examinations, we may determine that some patients require further evaluation in the emergency department or direct admission to the hospital.
